Ethiopian Origin ID Card Requirement and Respective Forms

1. Please read the following carefully and fulfill all the requirements. Otherwise, the Consulate will not be able  issue new passports  nor to process your request for passport renewal.

1. Application Form
2. Two  recent color passport-size photographs 1.552 inches
3. Copy of Green card (both sides) or I-94 /INS papers
4. Notarized Finger Prints
5. Old Ethiopian Passport and supportive documents issued by the government of Ethiopia, if applicable
6. $50.00  must be in paid by money order/cashier's check payable to the Consulate General of Ethiopia.
7. A pre-paid express/ registered/certified envelope for return mail

5. Old Ethiopian Passport and supportive documents issued by the government of Ethiopia, if applicable
6. $60.00 must be in paid by money order/cashier's check payable to the Consulate General of Ethiopia.
7. In case of a lost passport, the loss should be reported to the police station and a letter/police report from the station verifying this should be sent to the consulate.
8. A pre-paid express/ registered/certified envelope for return mail

4. Renewal fee $50.00 in money order
5. A pre-paid registered/certified envelope for the return mail.
